USO Illinois wants to help you celebrate the summer! Complete the registration and you may be randomly selected to receive a personalized USO Operation Summer Surprise kit filled with summer fun recreational goods! The pack will be delivered to your home by a USO team member or volunteer.

Submissions will be taken until July 15, 10 a.m. CT

Deliveries will be made from July 4, 2021 - Labor Day 2021
